Terms And Conditions

I have read and agree that:

• All details in this application are true and complete.
• Within the first 30 days of my pet’s enrollment in a Pets Plus Us pet insurance policy, I will provide all medical history of my pet including but not limited to:
  • Examination notes, laboratory reports, and referrals from all treatment providers for my pet for the past 18 months.
  • The records will include a complete physical exam by a licensed veterinarian within the past 6 months, or if not available, a complete physical exam within the first 14 days of coverage.
• My pet’s policy has specific waiting periods that must be satisfied before coverage may be available.
• There is no coverage available for pre-existing conditions, whether a diagnosis has been made or not.
• Claims must be received by Pets Plus Us no more than 6 months from the first date of treatment.
• Additional enrollment requirements must be met if at the time of enrollment my pet is 8 years or older (6 years or older for giant breed dogs). I am responsible for reviewing my Pets Plus Us insurance coverage User Guide as it outlines my pet’s coverage details.
• Premiums are not fixed and may increase over time.
• Coverage may be denied and/or the policy might be cancelled if there is a discovery of fraud, concealment, or material misrepresentation. I am at least 18 years of age and the primary owner of the insured pet.

I now authorize:

• My veterinarian to release all medical information to Pets Plus Us, and confirm any medical details, as required.
• Pets Plus Us to advise my veterinarian of the coverage(s) I choose.
